Pension Fund forms may be downloaded, printed, completed and mailed or hand carried to the Pension Office at the address listed below. Unless otherwise specified, Pension Administration must have the original form on file in order for the form to be valid. (Faxed or e-mailed forms will NOT be accepted.) The Refund of Contribution application posted here is provided so that you may preview a copy of the forms that are required in order to withdraw pension contributions upon termination of employment. These sample forms are not official forms of the City Employees’ Pension Fund. The forms found on this site are “specimen” forms, and are for informational purposes only. The application should be completed in the Pension Administration Office, and a pension administration staff member witnesses your signature. Please contact the Pension Administration Office to schedule an appointment to complete an application, or to make other arrangements if you are not able to come into the office.
Open Comment Period
During the Open Comment Period of the meeting, audience members who sign up will be given an opportunity to address the Board. Unless modified by the Chair, speakers will be allowed three (3) minutes for their comments. If you wish to address the Board during the Open Comment Period, please complete the Audience Participation Sheet. Forms can be submitted by email to firstname.lastname@example.org. Forms can also be mailed or delivered in person to the Pension Office located at 400 W. San Antonio Ave., Suite B, El Paso, TX 79928.
